- Partnering for Adams County Kids (PACK) Referral
- Partnering for Adams County Kids (PACK) is a 2Gen approach and multi-system integrated planning program for Adams County children, youth, and families with children and/or youth in the home from birth to 21 years of age. PACK is a collaborative meeting that includes children/youth, parent/guardian/caregivers, and representatives from community agencies in Adams County who support youth and families. The goal is to produce an action plan that will connect the family with vital resources to improve outcomes and increase service connection for Adams County families. This program serves Adams County residents with a child in the home from birth to 21 years.
Upcoming Meetings
The Adams County Collaborative Management Program (ACCMP) meets bi-monthly, on the second Tuesday of the month from 2 - 4 p.m. at the Adams County Human Services Center.
The Adams County Collaborative Management Program (ACCMP) Executive Committee meets monthly, on the first Wednesday from 12 pm-1:30 pm, virtually.
